Please note: this form is only for the use of Program Managers of Out-Wayne CoC funded housing programs. If you are in need of homeless or housing services, please follow this link to fill out a referral form or call our Coordinated Entry line at 313-388-9799 for assistance. 

For programs: please submit any housing program vacancy requests via this form to receive referrals from the Out-Wayne Coordinated Entry's By-Names-List. Be advised that if your organization receives CoC funding, your organization must use the Coordinated Entry system.

Within three business days of receiving your request, Coordinated Entry will make referrals through HMIS. After referrals are sent, your program will have three business days to respond to the referral(s) within HMIS.
