Background and Information

Purpose

The Out Wayne County Continuum of Care (CoC) is committed to carrying out a process for prioritizing and ranking renewal projects in a manner that is objective, upholds compliance with HUD requirements, and ensures projects are in line with CoC priorities and thresholds. To this end, the CoC has developed an Intent to Renew (ITR) application for CoC funded projects. With this being the inaugural year of this process, the CoC hopes responses received will provide an opportunity to provide projects with TA if thresholds are not met and also provide a baseline to build upon and reference in subsequent years. If TA is unable to resolve serious issues such as significant performance or financial issues, insufficient participation in HMIS, and other priorities of the CoC, the CoC may decide to not renew a project.

The ITR is also an opportunity for projects to provide the CoC notice of whether they intend to renew and continue operating. 


This process is not meant to place additional burden on the projects, and should be viewed as the first step in the Notice of Funding Opportunity (NOFO) process.

Monitoring Self-Assessment

The Monitoring Self-Assessment portion is designed to prepare projects for upcoming monitoring activities in the coming year. Because this is the first year the assessment is being introduced, the project’s assessment score will not impact their renewal application. However, it can serve a baseline for measuring future performance and may identify areas for Technical Assistance from the CoC team.

Technical Assistance (TA) Support

If results from the Monitoring Self-Assessment identify any areas where the project does not meet compliance or threshold requirements, the CoC team will reach out to the project contact listed to schedule TA support.
